When Blakeford at Green Hills was started in 1996 by a group of local citizens, they never imagined it would become Nashville’s premier provider of senior lifestyle solutions. Their goal was to offer a happy, healthy and safe home for their family members and other loved ones.
That they did.
Blakeford at Green Hills Life Plan Community
The nonprofit Life Plan Community offers a wide range of lifestyle options and health services matched to each resident’s individual and changing needs to encourage independence, individuality, and privacy. Locally managed, Blakeford at Green Hills has been serving senior adults and their families in Nashville for more than twenty years.
Home Options with Blakeford At Home and LiveWell By Blakeford
Several years ago, the visionaries at Blakeford saw the need for quality care for seniors living at home. They leveraged the expertise of staff members and the reputation of the Blakeford name to start, first, Blakeford At Home and, then, LiveWell By Blakeford.
Blakeford At Home offers care for those who need a hand at home. LiveWell By Blakeford is based on the continuing care model, providing services at home to healthy, independent older adults initially, as well as the assurance of continuing care should they need it.

Accreditation
Blakeford at Green Hills and Blakeford at Home are accredited through CARF-CCAC (Commission on Accreditation of Rehabilitation Facilities-Continuing Care Accreditation Commission). Blakeford is now one of fewer than 200 Life Plan communities in the country to have achieved this designation and, currently, the only accredited provider in the Middle Tennessee area. This accreditation makes Blakeford At Home the only provider of non-medical home and community services in the state of Tennessee to carry this distinction.
Awards
2019 – Burton Court at Blakeford (Assisted Living), received the prestigious Silver Quality Award from the American Healthcare Association and National Center for Assisted Living (AHCA/NCAL).
2019 – Blakeford at Green Hills and Blakeford At Home received a second consecutive five-year accreditation from CARF (Continuing Care Accreditation Commission).
2018 – Blakeford’s IT Department was awarded the “Excellence in Innovation” from LeadingAge Tennessee, for the creation of the resident intranet.
2018 – Blakeford’s Wellness Department received the “Honorable Culture of Wellbeing” award among the Mid-South division of Cigna clients for creating an environment of health and wellness for Blakeford’s team members.
2017 – Our skilled nursing center, Woodcrest at Blakeford, received the prestigious Silver Quality Award from the American Healthcare Association and National Center for Assisted Living (AHCA/NCAL).
2017 – Chaplain Sherry Perry and Blakeford Friends won LeadingAge Tennessee’s Excellence in Volunteerism award.
2014 – Blakeford at Green Hills and Blakeford At Home are awarded a five-year CARF-CCAC Accreditation demonstrating a dedication to creating a culture of exceptional quality and service for residents.
